Consult a licensed Missouri physician

Physician certifies your condition
The physician submits a certification to the state confirming you have a qualifying condition.
Create your state account
Register at mo.gov/myDHSSportal to begin your application.
Submit application + certification
Upload your physician certification, ID, and complete the application form.
Pay the $25 application fee
One-time fee. Cards are valid for 3 years.
Receive your card
Typically processed within 30 days. Card is valid for 3 years from issue date.
